Dracut Warning: could not boot, /dev/"hard drive" does not exist

When booting IPFire (core 131 x86_64) it will occasionally fail to boot and drop to the debug shell after giving a dracut warning that the hard-drive can't be found, in this case /dev/mmcblk0p4, giving an error message that I've attached a picture of. Checking the boot commands the root is pointed towards the right partition (\dev\mmcblk0p4).

Restarting once or twice will usually fix this and boot properly although sometimes it takes more than that. I have never been completely unable to boot due to this error as eventually it will recognize the hard-drive and boot. It seems to to have started with Core 131 but the only thing I can say for sure is that I never saw it in Core 127 before updating straight to Core 131.
